Enrollment Requirements for Junction City Middle School

• Proof of Immunizations

• Proof of Identification (Birth Certificate)

• Social Security Number (if available)

• Certified transcript or similar record or data of previous school

• Attended (most recent grade card)

• Current IEP (Individual Educational Plan) if applicable

     If you can, an official copy of the student’s records can be hand carried.  You would need to notify your current school         

     of this request in advance and the records must be sealed by the previous school to be considered official.
